I am not pregnant or breast feeding. I am not taking Roaccutane. I understand if I have an extensive medical history I should contact Cosmetic FX prior to booking.

I understand if I have a previous tattoo I MUST send in photo's (email to info@cosmeticfx.com.au) prior to booking an appointment to confirm they can go over my existing tattoo.
